Dr. Shiffman is no longer seeing patients, renewing prescriptions, taking or returning telephone calls, providing medical advice, or filling out forms for patients.

REQUESTS FOR MEDICAL RECORDS

Dr. Shiffman and his administrator will be out of the office, and out of the country, from August 20 until September 20.  WE WILL NOT BE ABLE TO RESPOND TO REQUESTS FOR RECORDS BEFORE SEPTEMBER 20.  YOU CAN MAIL OR EMAIL THE REQUEST, BUT PLEASE DO NOT EXPECT A RESPONSE UNTIL AFTER SEPTEMBER 20.

Under HIPPA, we can only send out records if we have a signed  HIPPA form from the patient. Note that we can send the records to the patient or to another provider.  You can print the request form by clicking here:

Fill out the request, sign it, scan it, and email it to:  admin@summitintmed.com. 

You may also mail it to:

If you mail it, please provide an email address to which we can email the records. 

INSURANCE AND BILLING:

Please continue to mail your payments to our post office box.
